Look at the lower right-hand corner on the back cover of your binder. There is a number there like “P010410.” This is a combination of the print number (01) and the date (0410 = April 2010).
If your binder is P010410, you need all the updates.
If your binder is P020510, you already have all of the forms (Appendix H). Other than Appendix F and the content of Chapter 3, Part S (see first paragraph on this page), you need everything else. You can either download the Zip file from CMS and throw away the parts you don’t need, or use the dated links below.
If you prefer, you can buy the printed updates from AHCA rather than download them. Updates are $14.95 + S&H. There are different sets of updates depending on your binder number: for P010410 order product #5011RA (entire contents + new tabs for forms) and for P020510 order product #5011RM (entire contents except for forms and tabs, which are already current in your binder). Call 800-321-0343 to order. Please note that we have printed limited quantities of the updates, and when we run out you will have to purchase a new binder to get the most current version.
Forms (Appendix H)
You can download the forms in two formats. Interactive Forms — This means that if you are asked to fill in information on the form, you can type information directly into fields in the PDF file. You can copy any or all of these interactive forms to your own computer. All the complete interactive forms are here. Forms to print — These are the same forms, but without the highlight colors in the interactive forms. You could also print the interactive forms, but they might not come out as well on a black & white printer. You can also download a chart of the changes made to the forms April 8, 2010.
Here are the latest files available from CMS. Note that each file has a version number (“v1.03”) and a date (“July 2010”). Because earlier versions were being released every few days in May, June, and July, they had a day in the date (“July 12, 2010”). Now there is only a month and a year (“July 2010”).
