A successful
application submission will generate two confirmations. If you do not receive
one of the following confirmations, your application may not have been
received. Remember, late applications are not accepted for any reason, so
please make sure you have proof of the following confirmations.
Confirmation
#1:
The first confirmation is a web page that populates after a successful
submission and reads “Application Confirmation.” As the message indicates, you
should print this screen for your records.

#2:
Once you submit your application, the system will immediately generate a
confirmation email from a mailbox called noreply@ahca.org. If you do not
receive this confirmation email within 1 hour (and you cannot find it in your
SPAM filter); please email quality-award@ahca.org. However, if this occurs
after the 8:00pm deadline, your application may not be accepted.

Award Application Upload Policy
- Applicants have the option of submitting their applications in PDF or Word format. The submission section of the following application form indicates word versions should be submitted. However, the system has been set up to accept PDF’s and Silver and Gold applicants are encouraged to submit PDF documents.
- The first application you submit is considered final.
- AHCA/NCAL reserves the right to disqualify any application that does not adhere to the technical requirements laid out on page 10 of the Silver and Gold application packets. Disqualified applications will not receive a feedback report or a refund of their application fee.
- Any application found to have plagiarized another will be disqualified, and the organization will be ineligible to apply for two subsequent years.
Silver Pre-Screening Policy (Silver Applicants Only)
All 2013
Silver applications are subject to a pre-screening process prior to being
forwarded to the Examiner teams for review. The pre-screening protocol will be
focused on the Organizational Profile and Category 7 (results). To be
successful at the pre-screen level, the application must demonstrate the
interrelationship between the Organizational Profile and Category 7; the
organization’s results. Applicants are encouraged to view the Silver Pre-Screening Form Examiners will use to determine whether your
application meets the pre-screening requirement. We encourage you to complete this
worksheet to your own application, to ensure your application passes the
pre-screening process.
