- Bronze Award Application Deadline – February 15, 2012
- Silver Award Application Deadline – March 1, 2012
- Gold Award Application Deadline – March 1, 2012
Application Support Hours
Quality Award program staff are available to answer applicant questions from 9am – 5pm EST, Monday through Friday, via email at quality-award@ahca.org.
Confirmation Email
Once your application is successfully submitted, the contact person on the application will receive a confirmation email. This email is extremely important, as it is confirmation of a successful submission. If you do not receive this email, please contact Quality Award program staff to verify your submission.
Quality Award Application Upload Policy
The first application you submit is considered final. Any additional applications uploaded after your original submission will be discarded. There are no exceptions to this policy, please make sure the application you upload is the final version.
