Gold Recipients 

Applicants who receive the Gold award are recognized as some of the best performers in the long term care profession. They are prior recipients of the Bronze and Silver awards. Applicants who receive the Gold award address the Baldrige National Quality Program Health Care Criteria for Performance excellence to demonstrate by approach, deployment, and level and consistency of results that they are achieving high levels of performance in health care, customer satisfaction, financial, market, workforce, process, and leadership outcomes over time. Independent award examiners conduct a site visit to verify systems and performance and the final decision is made by a group of Master Examiners.

  • “Life Enriched” is the motto that helps staff to remember that each life is precious and guides them in providing exceptional care. This year, 45 of the facility’s 300 staff members celebrated 20 or more years of service.
  • Clement Manor helps people of all ages in the community age in a healthy way. Young children come to Child Day Care and the Center for Enrichment and Life Long Learning provides diverse educational, social, wellness and spiritual growth opportunities for Clement Manor residents as well as more than 500 older adults in the local community. Memory care programming is coordinated through the continuum including brain fitness courses, an early memory loss program, and a specialty unit within skilled nursing.

  • Nearly half of Lakewood’s 278 dedicated staff members have been working at Lakewood for more than 5 years.
  • With strategic investments in advanced equipment and treatments, such as Doppler Ultrasound Exams and EKGs, Lakewood performs high level testing, increasing resident comfort and reducing unnecessary hospital stays.
  • Since 2003, Lakewood had been recognized several times for their innovative practices and unique programs; Pay It Forward, a staff recognition program that honors 50 to 60 staff members a month; ElderBridge, a customer service program that helps address and resolve resident concerns; and Seasons(SM) Memory Care, a program specially designed for residents with dementia, contribute to the high level of quality care provided by this award winning facility.
  • Since earning the AHCA Step I Quality Award in 2004, Lakewood has achieved two important milestones. In 2005, the facility was the first in New York State to achieve Step II. This year, it is the first to reach Step III.
  • Validation of Lakewood’s dedication to continuous quality improvement are their resident/family overall satisfaction and recommendation to others scores. In fact, in their most recent survey, 95% of residents and families report overall satisfaction and would recommend Lakewood to others*.

  • The core values of this organization, Hospitality, Stewardship, Justice and Respect are lived daily by a devoted staff of 170 and led by a management team averaging 17 years longevity.
  • Villa St. Vincent has 104 beds including a Memory Care Unit dedicated to dementia and Alzheimer’s care in a resident centered, home-like environment.
  • The Summit and Good Shepherd Hall together offer 76 assisted living beds and are complimented by their tranquil settings, customized amenities and competent staff.
  • Seeking and responding to the voice of residents, families, staffs and medical providers is one key to success. This information is used during annual strategic planning and work process improvements.

  • St. Benedict’s staff and leaders are always on the lookout for ways to improve the quality of life for the residents and families we serve, through strategic visioning, process improvements, data driven decision making, better training and staff engagement, facility renovations, and higher standards of customer service.
  • Our campus offers a continuum of care, including skilled nursing, assisted living, child day care, and most recently, PACE (Plan of All-inclusive Care for the Elderly). Through a partnership with Northland Healthcare Alliance, we are one of two sites in ND, and one of 7 rural PACE sites nationally, serving people who are eligible for nursing home care but wish to – and with PACE support are able to - stay at home.
  • St. Benedict’s has incredible support from the community that we serve and our many friends. Over 400 volunteers help us on an annual basis, with things such as resident activities, decorating, helping with fundraisers, mailings, and so many other tasks. We also have wonderful philanthropic support, which has allowed us to make some remarkable improvements throughout our facility.
