At the AHCA/NCAL Quality Symposium, it was announced that LTC Trend Tracker now has an MDS 3.0 rehospitalization measure - OnPoint-30® - available for skilled nursing facilities.
Provided by PointRight, this new report allows users to benchmark hospital readmission rates against peers (from zip code to the national level). In addition, OnPoint-30 provides actual, expected and risk-adjusted rates, allowing users to furnish hospitals with information that may impact referral patterns.

Five Star QM Rating Report allows a user to determine which QMs to work
on to increase their QM Score and possibly their Five Star rating.
This is the first LTCTT report that allows users to directly input
information and use the system for goal setting. The user can decide
which QMs they need to focus on as an organization to get to the next
level on the QM Five Star Rating.
Other upgrades include a NEW QM Report and enhanced printing of the
dashboard.
