Population Health Management Summit

​AHCA/NCAL’s Population Health Summit (PHM) is the only gathering of its kind specifically focused on the long term care and post-acute care community. With CMS’s ongoing focus on shifting from fee-for-service (FFS) to accountable care arrangements, prioritizing value-based care is increasingly essential for the long term care industry. The PHM Summit serves as a hub for innovation and thought leadership, featuring a diversity of sessions tailored to both newcomers to PHM and seasoned providers accustomed to managing risk.

Networking is a key aspect of this interactive event: attendees will have ample opportunity to make connections with and learn from other providers as well as engage with industry experts who will discuss various population health models including ACOs, I-SNPs, assisted living and value-based care, provider-owned networks, and emerging models. Attendee Eligibility
Participation in this conference is intended for long term care (LTC) member and nonmember providers, AHCA/NCAL state affiliate staff members, nonmembers other than providers (e.g. health plan staff, consultants, and attorneys), and other nonmember association and not for profit community organizations. AHCA /NCAL reserves the right to grant final approval to all registrants. Please note CE accreditation is available via this conference’s education sessions.

Conference Registration Payment
Payment fees for the conference and optional activities can be made by U.S. check, American Express, Visa, or MasterCard. AHCA/NCAL will not process your registration without full payment. To avoid duplicate charges to your credit card, DO NOT mail a copy of your registration form once it has been faxed or posted online.

Substitutions
Registration substitutions will be allowed up to five (5) days prior to the conference. Changes and/or substitutions must be made in writing via the PHMS Registration Support.

Cancellations/Refunds
Cancellations received on or before Thursday, May 9, 2024, will be refunded minus a $75.00 administrative fee. Any change or cancellation must be made in writing. Refunds will not be issued after Thursday, May 9, 2024. Refunds will be processed within five (5) days following the end of the conference. Refunds or credits will not be made on no-shows.

Special Accommodations and Dietary Needs
We do our best to include vegan and gluten free option at every meal. If you require special accommodations or have special dietary needs, please contact AHCA/NCAL at meetings@ahca.org.

Conference Attire
Business casual attire is suitable for all meetings, sessions, and receptions. Please remember to bring a sweater or jacket for air-conditioned rooms.

Make Your Reservation Early!

Take advantage of the special conference room rate. Make your reservation by Thursday, May 9, 2024. Reservation requests made after this date will be based on availability at the prevailing hotel rate. The hotel requires a first-night deposit of room and tax to guarantee accommodation.

Convention Management Resources Inc., (CMR) is AHCA/NCAL’s Quality Summit official registration and housing company. Please register and make your hotel reservation online. Conference registration is required to secure your hotel reservation.

Room Rates & Taxes

AHCA/NCAL has arranged for the special group rate of $239.00 per night single or double occupancy plus appropriate tax and service charges. This rate will be extended by three days pre- and post-conference dates, based on availability.

 Health & Safety

With health and safety remaining a priority for our attendees, we ask that the following be observed:

  1. Being up to date with current vaccines/boosters is preferred for all registered attendees.
  2. Masks are worn at the discretion of the attendees. 
  3. Do not travel if you have COVID until you meet the CDC criteria for returning to work for Healthcare Workers.
  4. If you have respiratory symptoms, please test for COVID before traveling and use a source control mask when around others indoors. 
  5. If you have had a recent high-risk exposure in the 10 days before a meeting or event, you should wear a mask for 10 days following the exposure and not participate in indoor activities where you may need to take off your mask (e.g., dining, drinking, coffee, etc.). High-risk exposure at work does not count if appropriate PPE was used during resident care.
  6. Follow all state, local, and private business health and safety measures that may be in place. 
  7. If you experience symptoms or are exposed while onsite, please self-quarantine in your hotel room and contact AHCA/NCAL staff (meetings@ahca.org) for a test kit.

About the Speaker:
Jeremy Utley is the Co-Founder of Stanford's Masters of Creativity at the Hasso Plattner Institute of Design at Stanford. He was formerly the Director of Executive Education at the Stanford School, where his blend of on-your-feet thinking and penetrating insight has earned him a reputation as a go-to advisor for CEOs and start-up founders alike. Utley, the co-author of the book Ideaflow (2022), and co-host of the popular Paint & Pipette Podcast, continues to work on invention, discovery, and entrepreneurship and demystifies the counter-intuitive techniques that drive productive creativity.

Known for his work in Lean Start-Up and Design Thinking methodologies, Utley shows their ability to drive product development and consumer engagement at a fraction of typical product development costs. Utley is also a General Partner at Freespin Capital, a venture firm that helps legacy companies launch hyper-growth start-ups. He holds a BBA with Honors in Finance from The University of Texas at Austin (2005) and an MBA from the Stanford University Graduate School of Business (2009).
