AHCA/NCAL PHM Summit Offers Unique Structured Networking Sessions

If you have not yet registered for the Population Health Management Summit, now is the time. It takes place December 6-7 in Washington, DC, and, in addition to excellent guest speakers and timely education sessions, the PHM Summit offers a unique structured networking opportunity in which you can meet and talk to people who are thinking about and getting involved in the same areas of PHM as you. It’s a great way to learn from one another, establish connections, and gain some valuable contacts as you move forward on your PHM journey. 

The Structured Networking Sessions take place on day one of the summit. Attendees will have three options from which to choose. 

OPTION 1: Meet with SNP Administrative Partners 
Facilitated by Amy Kaszak, President, Special Needs Plans, AllyAlign Health, Rick Grindrod, President/CEO, Provider Partners Health Plan, Marc Hudak, Chief Growth Officer, Longevity Health Plans, and Hank Watson, Chief Development Officer, American Health Plans. 

Meet and speak with the organizations that are partnering with SNF and AL providers to propel and support provider-owned special needs plans. Ask questions to understand your options more fully for ownership and/or collaboration. 

OPTION 2: Mature Provider Led SNPs: You are Not Alone 
​​Facilitated by Richard Brockman, SIMPRA Advantage. 

Join your peers to share successes, lessons learned, challenges, ideas, and next steps concerning provider-owned special needs plans. 

OPTION 3: Provider-Owned Networks 
​​Facilitated by Ted LeNeave, Chief Executive Officer & Founder, Accura HealthCare and Lisa Toti, President, Accura HealthCare. 

This networking session is for those that participate or have an interest in provider-owned networks, which are advancing opportunities and outcomes for value-based care contracts across multiple states and growing.
