Failure to comply with Skilled Nursing Facility (SNF) Quality Reporting Program (QRP) data submission requirements may result in a 2-percentage points reduction to the market basket update (also known as the Annual Payment Update).
AHCA is aware that some facilities have been experiencing issues accessing the HPS component in NHSN to report the SNF QRP required data on annual HCP influenza vaccination.
If your facility was not able to submit the required SNF QRP data by the May 15th 11:59 PM deadline, CMS provides SNFs an opportunity to request a Skilled Nursing Facility (SNF) Quality Reporting Program (QRP) Reconsideration and Exception & Extension from the program’s reporting requirements in the event the facility was unable to submit quality data due to extraordinary circumstances beyond their control. SNFs affected by a natural or man-made disaster or other extraordinary circumstances may request an exception or extension by filing a Request for Reconsideration Due to Disaster or Extraordinary Circumstance.
The request must be submitted via email to
SNFQRPReconsiderations@cms.hhs.gov with the subject line, “Disaster Exception or Extension Request,” within ninety (90) days of the event and must include the following information:
- SNF CMS Certification Number (CCN)
- SNF Business Name
- SNF Business Address
- CEO or CEO-designated personnel contact information including name, email address, telephone number, and physical mailing address (no post office boxes). Note: This may be the Administrator.
- Description of the event associated with the reason for requesting the exception or extension
- A date when the SNF believes that it will be able to submit SNF QRP data and justification for the proposed date
- Any other documentation supporting the rationale for seeking reconsideration
CMS will send an email acknowledging receipt of the exception and extension request and will notify the CEO or CEO-designated contact person via email with a final decision.
