CMS Announces Reformatted Statements of Deficiency

During the recent 2025 Quality Conference, the Centers for Medicare and Medicaid Services (CMS) announced its intention to reformat Nursing Home Statements of Deficiency.  Director of the CMS Quality, Safety, and Oversight Group, David Wright, shared that the agency plans to update how deficiencies are presented in these documents. In the coming months, CMS will begin listing deficiencies with the highest scope and severity at the beginning of the Statement of Deficiencies. 

Currently, deficiencies are listed sequentially by their associated F-tag. CMS noted that this upcoming change aims to emphasize the most serious findings by placing them first in the document.
