 Databooks

Discharge to Community:
These databooks provide the rate at which all patients who enter skilled nursing facilities (SNFs) from acute hospitals and are subsequently discharged back to the community within 100 days of admission. These patients remain out of any skilled nursing facility for at least 30 days. They provide the actual, expected, and risk-adjusted discharge to community rates.

Analysis Tips:

  • When comparing the discharge to community performances of two facilities, their adjusted rated should be used.
  • When assessing changes over time in a single facility’s performance, the actual rate usually is more useful.
  • Please refer to the discharge to community help doc for more details.

Length of Stay (LOS):
These databooks provide the median length of stay for all new admissions from a hospital as well as percentage of new admissions who have stays of 7 days or fewer, 14 days or fewer, 20 days or fewer and 45 days or fewer.

Analysis Tips:

  • LOS is not a measure of quality and has no targeted goal, but some consider it a measure of efficiency.
  • LOS must be interpreted in the context of quality measures such as rehospitalization and discharge to community.
  •  Please refer to the length of stay help doc for more details.

PointRight (R) Pro 30 Short Stay Rehospitalization:
These databooks provide the rate at which all patients who enter skilled nursing facilities (SNFs) from acute hospitals and are subsequently rehospitalized during their SNF stay, within 30 days from their admission to the SNF. They provide the actual, expected, and risk-adjusted rehospitalization rates.

Analysis Tips:

  • When comparing the rehospitalization performances of two facilities, their adjusted rated should be used.
  • When assessing changes over time in a single facility’s performance, the actual rate usually is more useful.
  • Please refer to the rehospitalization help doc for more details.
