State Executive Member $325 per person​

Attendee Eligibility

Attendee registration for this event is exclusively for long term care and post-acute care providers. Registration and attendance for companies and individuals who sell or market products and/or services to provider organizations is limited to sponsors only. AHCA/NCAL reserves the right to grant final approval for all registrants to attend this conference.


General Information


Registration Payment

Payment for the conference can be made by U.S. Check, American Express, Visa or MasterCard and must be submitted with your registration. AHCA/NCAL will not process your registration without full payment.

To avoid duplicate charges to your credit card, DO NOT mail a copy of your registration form once it has been completed online or faxed. Conference registration confirmations will be e-mailed to you within three days of receipt.​

Substitutions

Substitutions will be allowed up to five (5) calendar days prior to the conference. Conference registration changes and/or substitutions must be made in writing via fax to 415-293-4743, Attn: AHCA/NCAL Registraiton or email Quality Summit registration.


Cancellations/Refunds

Cancellations received on or before Friday, Februay 2, 2018 will be refunded minus a $75.0​0 administrative fee. Any change or cancellation must be made in writing via fax to 415-293-4743, or e-mail Quality Summit registration​.


Refunds will not be issued after Friday, February 2, 2018. Refunds will be processed within 15 days following the end of the conference. Refunds or credits will not be made on no-shows.


Conference Attire

Business casual attire is suitable for all meetings, sessions, and receptions. Please remember to bring a sweater or jacket for air-conditioned rooms.​
