AHCA/NCAL has been notified that a vital component in ensuring that your Point-of-Care Testing Result information is submitted correctly lies withing the accuracy of the CLIA identification number listed for the facility in the National Healthcare Safety Network (NHSN) system. Without an accurate CLIA ID number your facility data cannot be submitted to your State Health Department.
The CLIA ID must fit this format:
- The first two numbers identify the state the lab is in.
- The third number is actually the letter “D”.
- Following is seven more unique numbers.
- There should be a total of 10 characters/digits for the CLIA ID.

AHCA/NCAL is encouraging providers to have the NHSN Facility Administrator check the CLIA identification number listed in NHSN for accuracy. If needed, facilities can follow the steps below to correct and update the CLIA ID number in NHSN.
- Click on "Facility" then "Facility Info" in the left navigation screen.
- The CLIA number will be listed on the right side of the facility information screen. Edit if needed.
- Then click on “Update” at the bottom of the screen.
