Washington, D.C.

Last week, CMS and the Centers for Medicare & Medicaid Services Innovation Center (CMMI) unveiled a sweeping “refresh” to their 10-year strategic plan highlighting its goal to shift all Medicare and the majority of Medicaid beneficiaries out of fee-for-service (FF
S) and into value-based payment and alternative payment models  with a special emphasis on accountable care.

Jonathan Blum, Principal Deputy Administrator/Chief Operating Officer of CMS (pictured on left) will speak at the PHM Summit about this broad vision for population health management (PHM) as well as CMMI’s key priorities as they relate to long term and post-acute care.

Additionally, Tim Engelhardt, Director, CMS Medicare-Medicaid Coordination Office (pictured on right) will discuss how states are approaching Medicare integration and review the potential implications for providers. He and fellow speakers, Alison Rizer, ATI Advisory, and Suzanne Gore, State Health Partners, will also talk about state trends, emerging models, and the potential rolesfor long term care providers to play in partnership with states and health plans.

It’s a critical time for you to find out what you can do to get started now. At the PHM Summit, you will gain a deeper understanding of PHM and what’s involved in value-based care, begin forming the necessary partnerships, and lay out strategic plans of your own.
