Lumina Care delivers
coordinated, preventative clinical programs for skilled nursing, long-term care, and post-acute care providers across the country. A recent winter storm demonstrated how vital our virtual services can be. When phone lines and WiFi were working, but roads were impassable, our remote clinicians delivered care, managed changes in condition, and kept patients from transfers that would have been a logistical nightmare in dangerous conditions.
Our After-Hours Telehealth program has helped providers treat approximately 98% of residents in place and achieve average rehospitalization rates of 2%. With our Transitional Care Management (TCM) program, providers have seen post-discharge rehospitalizations reduced by 50% within five months of implementation.
Patient-centered care is at the center of everything we do. Lumina Care makes it easier for providers to implement CMS-backed behavioral health programs, including Behavioral Health Integration (BHI), Collaborative Care Management (CoCM), and Telepsychiatry, delivered remotely by board-certified psychiatrists and mental health nurse practitioners. When behavioral health is integrated into daily care, residents receive more timely support, reliance on antipsychotics and high-risk medications is reduced, and physical health outcomes improve alongside mental well-being.
In addition to our virtual clinical programs,
Lumina360 adds operational confidence for administrators, care teams, and corporate offices. By integrating with PointClickCare and other EMRs, Lumina360 helps providers monitor KPIs, care delivery, and trends at a glance. In a click, users can view individual patient data to identify care opportunities and facility-wide overviews to track compliance tasks.
