CMS Releases Technical Specification for Updated SNF QRP Falls with Major Injury Measure

The Centers for Medicare & Medicaid Services (CMS) has released the official Technical Specification Report for the Falls with Major Injury (FMI) measure. This report incorporates feedback received during the cross-setting Technical Expert Panel (TEP) held on May 12 and 14, 2025.  

The report provides: 

  • An overview of the measure, 
  • A description of the methodology used to construct the FMI measure, and  
  • An overview of measure testing results.  

It is important to note that only the measure under the SNF Quality Reporting Program is impacted by these changes. There is no change at this time to the long-stay Falls with Major Injury measure included in the Five-Star.  

What is Changing? 

The updated SNF QRP measure will continue to use MDS assessment data and add Medicare fee-for-service claims data to identify falls with a major injury that occurred during a Medicare Part A SNF stay. Claims will include those for inpatient hospital stays, emergency department visits, and observations stays. 

Additional guidance and related updates to the SNF QRP Quality Measure Calculations and Re​porting User’s Manual and public reporting timelines will be provided by CMS at a future date.
