PHM Innovation Lab Subject Area Modules 4-6 Events Published:September 23, 2020 Nisha Hammel Page ContentThe PHM Innovation Lab delivers the building blocks to educate long term care and post-acute care (LTC/PAC) providers about the different PHM models and opportunities for participation and leadership within the various PHM models. The PHM Innovation Lab's next three subject area modules with corresponding educational components including webinars and briefs are highlighted below. Emerging Models: Direct Contracting -- Provides a detailed overview of Direct Contracting including types of participants, payment, beneficiary alignment, quality incentives, and a model timeline. This module contains a shorter three-page brief covering Direct Contracting and a more detailed six-page summary. Managed Care -- Provides an overview of Medicare Advantage/ Provider-led Special Needs Plans (SNPs) and explains how SNPs work, and discusses the implications for LTC and PAC providers. This module contains a 48-minute webinar and three briefs on various topics totaling 30 pages.Provider Networks --Provides an overview of the why, what, and how of provider-owned integrated care networks and explains the benefits of provider networks. This module contains a 29-minute webinar and one-page issue brief. Members will need to login with their AHCA/NCAL usernames and passwords to access PHM Innovation Lab materials.
