The submission deadline for the Skilled Nursing Facility Quality Reporting Program SNF QRP) is fast approaching. This includes Minimum Data Set (MDS) assessment data and COVID-19 vaccination coverage for health care personnel submitted to the Centers for Medicare & Medicaid Services (CMS) via the Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN) for September 1 - December 31 (Q4) of calendar year (CY) 2025, along with the annual NHSN Influenza Vaccination among Healthcare Personnel measure data (for the 2025-2026 season). All data must be submitted no later than 11:59 p.m. on May 18, 2026.
It is recommended that providers run applicable Internet Quality Improvement and Evaluation System (iQIES)/NHSN analysis reports prior to each reporting deadline, to ensure that all required data has been submitted. Recommended iQIES reports include:
- The SNF QRP Provider Threshold Report, and
- The SNF QRP Review and Correct Report.
Swingtech sends informational messages to SNFs that are not meeting SNF QRP thresholds on a quarterly basis ahead of each submission deadline. If you need to add or change the email addresses of facility or corporate staff to which these messages are sent, please email QRPHelp@swingtech.com and be sure to include your facility name and CMS Certification Number (CCN) along with any requested email updates. At a minimum, it is recommended that a facility Administrator and Director of Nursing register for these emails.
